  • New covid variants: New covid variants (FLIRT) activate fear of an increase in cases in summer. The new variants of covid are spreading throughout the world.   • The Nuffield Trust (a major British health think tank) analyzes the changes in the NHS as a result of the increase in private activity: How private healthcare has changed British healthcare. In real terms, trusts (hospitals) spent £1.6bn in 2019/20; but this has risen to 3.12 billion in 2022/2023. Since Covid there has been a substantial increase in spending on the NHS purchasing private care.
  • Important hospital reform in Germany: Bankrupt hospitals in Germany approve a broad reform, which consists of the closure of part of the 1,700 hospitals, the most important reform in the last 20 years. Social Democratic German Health Minister Karl Lauterbach said there were too many hospitals and that Germany has neither the financial means nor the medical or nursing staff for that number. Its application will take about ten years and a transformation fund of 50,000 million euros has been allocated to implement it, half of which is borne by the federated states and the other half by the federal government.
  • Problems of private health insurers in Latin America: Both the ISAPRES in Chile (Boric government) and the EPS in Colombia (Petro government) are suffering very serious problems, which threaten their continuity.

  • The government proposes changes in private healthcare and its collaboration with the public system: The Ministry of Health puts out the draft Law on Public Management and Integrity of the SNS for public consultation. Summary

From the point of view of Biomedicine, the mega-study of 250,000 genomes made public by the National Health Institutes of the USA stands out. This is in line with carrying out genome studies combining it with electronic medical record data from very large groups initiated in the United Kingdom by Genomics England and also developed in Iceland and Japan. A very promising way to improve knowledge of many diseases. New treatments for type 1 diabetes, based on the transplant of insulin-producing beta cells, obtained from stem cells, neutralizing the immune reaction. Gene therapy is the great promise for many rare diseases, currently without treatment. The problem to be solved is the high price of the treatments. At the present time, saying breast or prostate cancer is almost saying nothing. The treatment of tumors is no longer based on their anatomical origin, but rather on their molecular characterization. Bispecific antibodies that act on two targets at the same time, useful in hematological and solid tumors. The FDA approves a drug to prevent the amputation of fingers and limbs.

As far as Global Health is concerned, long covid was understood as a sequel to covid, but an article in The Economist states that covid is not the only infectious disease that leaves pathological conditions that are difficult to label. An article in Health Affairs places the solution to the obesity epidemic as a joint effort of prevention and treatment.

In terms of International Health Policy, more than half of USA’s rural hospitals do not offer childbirth services, which means that pregnant women have to be treated in centers located 30-40 minutes from their home, sometimes more. This is related to the high maternal and infant mortality in the USA, higher than that of most developed countries. The 4th anniversary of Brexit encourages the debate about whether it was good or bad for the National Health Service. The truth is that the NHS is worse now than before Brexit (waiting lists, professional strikes, etc.) and the difficulty of recruiting health professionals from the EU has increased. The investigation was also damaged. Increasing funding for the NHS was one of the arguments used by Brexiteers. France addresses the carbon footprint in the health system, within the general “France Nation Verte” program. Germany legalizes recreational cannabis, with one of the most liberal laws in Europe, after Malta and Luxembourg. If we talk about National Health Policy (Spain), as a result of a specific scandal, the entire problem of purchasing masks and other materials during covid is introduced into the national debate. Precisely everything related to the purchase of these materials was somewhat overlooked by the report “Evaluation of the Performance of the Spanish National Health System in the face of the covid-19 pandemic”, dated April 30, 2023, although published by the Ministry of Health in December of that year. It is already serious that an issue such as the problems of purchasing materials are not analyzed in the report, as if this had not been one of the most serious problems in the management of covid. The PNV requests the return of the text of the Public Health Agency law. That this law was going to have problems with the nationalist parties, always opposed to the creation of cohesion bodies in the National Health System, was something known. Surely this will mean delays in the appearance of the law and, what may be more serious, affect its content and functions. Summary

From the point of view of Biomedicine, this week marked the 20th anniversary of the completion (later it turned out not completely) of the Human Genome Project, whose results were presented on April 14, 2003. A project that revolutionized biology and Medicine. Many of the treatments that are now in sight, such as gene editing, or the screening of certain diseases come from the progress of that project.

As far as Global Health is concerned, the criminalization of suicide (considering suicide attempts as a crime subject to fines or imprisonment) is still in force in more than 20 countries. Not so many years ago suicide attempts were decriminalized in advanced societies, as recently as 1966 in England or 1993 in Ireland. Clearly, what these suicidal individuals need is care and treatment.

As for International Health Policy, the United States continues to have problems with the Supreme Court’s decision to annul the FDA’s authorization of an abortion pill, a measure that was later put on hold. The crisis in the British NHS continues, with two important strikes called: nurses and junior doctors.

If we talk about National Health Policy (Spain), the incidence of covid has grown slightly in the last week. Medical and health conflicts continue in various autonomous communities: the Valencian Community, Galicia, the Canary Islands, Madrid and Andalusia. Great news, if confirmed, the arrival of the first publicly funded biomarkers on the SNS. It will start with those related to oncology. A breakthrough in precision medicine. Initiative from Aragon for a debate on health that is intended to be held at the Conference of Presidents. From the point of view of Biomedicine, the debate on the governance, supervision and ethical limits of human genome editing is increasingly present, precisely when it seems that many medicines based on this technique are in advanced development, to treat many sicknesses. Nature publishes a novel approach to disseminate Magnetic Resonance, which fifty years after its birth is still a very expensive technique and not accessible to many countries. The mapping of the brain of a larva, after decades of research, heralds advances in the knowledge of more complex brains.

As regards Global Health, the WHO has drawn up the first draft of a treaty on pandemics, precisely to avoid what happened during the covid, the great differences in mortality, morbidity and access to treatment between the different countries.

Regarding International Health Policy, in the United States, the New York Times denounces that aggressive medical behavior continues to be common at the end of life. This same newspaper echoes in its editorial section the serious crisis of the British National Health Service, where several strikes are coexisting at this moment: nurses, consultants, junior doctors and ambulance drivers. The prestige of the NHS among the population plummets. Le Monde publishes in France a report on the change of pharmacies: from drug stores to health “hubs”. With regard to Global Health, it should be noted that 839 million people around the world cannot eat with dignity, for various reasons; the magazine Nature calls for a treaty against new pandemics, since it estimates that the covid represented “a catastrophic failure of the international community in achieving solidarity and equity”. The malaria pandemic is still active in more than 30 countries .

Likewise, the minirobot designed by Apple capable of taking medicines to the brain, that organ until now practically inaccessible to drugs.

With regard to Global Health, the COP27 summit is considered in an article in The Lancet a “collective failure”, given the lack of agreement on the lower use of fossil fuels. We must also note the race for the production of vaccines in several African countries.

Regarding International Health Policy, mortality from covid remains high and is very worrying in some countries, mainly Japan, Brazil, India, China, the Russian Federation, South Korea and Spain. The covid-zero policy is generating problems and discontent in a country where public demonstrations are as controlled as in China. The nurses’ strike in the United Kingdom is a historic event in that country, which will undoubtedly will have consequences.

Summary

From the point of view of biomedicine, the announced “vaccine against cancer” should be highlighted, which could be prepared before 2030. Although not strictly a vaccine, it represents a promising treatment against this disease based on mRNA technology, the same used for covid vaccine.

Regarding Global Health, an important announcement by the WHO about the dangers of a sedentary lifestyle, which could produce 500 million chronic patients in 2030. The WHO dedicates 2,600 million in its next strategy 2022-2026 for the eradication of polio .

Regarding international health policy, an important debate introduced by The Economist on the massive use of antidepressants, when only in 15% of cases do they have an effect other than placebo. Opioids in the United States and antidepressants worldwide are examples of the misuse of medications, which can lead to, and in many cases lead to, health problems. The post-pandemic horizon has generalized a hybrid work model (face-to-face and online) for office professionals.

If we talk about national health policy (Spain), the pressure on hospitals due to covid remains low, despite a slight rise in this disease. Summary

From the point of view of biomedicine, immunotherapy (in colon cancer) and CAR-T therapies (in lupus) continue to expand their therapeutic possibilities. Advances against infections for resistant bacteria. Will there ever come a time when cancer can be diagnosed by a simple blood test? This possibility does not seem very remote, according to papers presented at the meeting of the European Society of Medical Oncology.

Regarding Global Health, an important work directed by Jeffrey Sachs and published by The Lancet on the delays and errors of both governments and the WHO in dealing with the covid pandemic, which could have produced 17.7 million of  deaths, many of them preventable.

Regarding international health policy, the WHO says that the end of the covid pandemic may be in sight, although at the same time a new variant (BJ.1) of covid is evident in Europe. Important debate in Europe (Scotland and France) on euthanasia and assisted death. The French Ethics Committee opens the door to this possibility. The financing of the American FDA was questioned, due to its great economic dependence on the pharmaceutical industry. SUMMARY

From the point of view of biomedicine, the progress in the treatment of breast cancer should be highlighted, according to the article published by the NEJM, with a brake of around 80% of this advanced cancer and makes it disappear in 16%. .

From the point of view of Biomedicine, the Lancet report on risk factors in COPD (chronic obstructive pulmonary disease), one of the great burdens of all health systems, should be highlighted. Also the finding that COVID can cause brain damage, although its scope is yet to be determined.

As for Global Health, it is worth noting the WHO complaint that 16 health facilities have been attacked in the war in Ukraine, as well as the organization’s statement that women should have access to safe abortions, to avoid mortality as well as hospitalizations due to complications.

When it comes to international health policy, the advice for the next pandemic contained in a major editorial in the Financial Times is important. There would be three priorities: 1. Reduce the risk of the appearance of a new pathogen (controlling the markets where animals are sold, for example); 2. Prevent it from spreading throughout the world; and 3.
