SUMMARY

From the point of view of biomedicine, it is worth highlighting the fact, highlighted by the journal Nature, that the WHO took more than two years to recognize the airborne transmission of covid. A new generation of vaccines that prevent cancer can make tumors disappear before they appear, according to an article in Science. The natural experiment from South Korea and Hong Kong shows that the “omicron” variant can be fatal in unvaccinated people.

As far as Global Health is concerned, the situation in Ukraine gives us difficult lessons in global health, explains the editor of The Lancet. It should be noted that the black American population already exceeds the white population in mortality from opioids. Until now, deaths from this cause occurred mainly in “non-colleged educated” whites.

Regarding international health policy, the European health agencies (EMA and ECDC) say that it is too early for the second “booster” of the covid vaccine. Ten years of “Choosing Wisely” the revolution in medical practice that tries to minimize worthless or low value health interventions.

If we talk about national health policy (Spain), the appearance of the 2022 Health Barometer (First wave) is important. It should be noted that waiting lists are the main concern of the population; the delay in a high percentage of cases to see a primary care physician; and that the positive perception of the public system decreases, while that of the private system increases. Report on health professionals, which says that within five years there will be a shortage of 9,000 doctors.

In the field of companies, from the international point of view, Moderna’s agreement with the non-profit entity IAVI to apply mRNA technology to various diseases should be highlighted. SpainCares confirms that Spain is not in an advantageous position with regard to health tourism.

SUMMARY

From the point of view of biomedicine, the progress in the treatment of breast cancer should be highlighted, according to the article published by the NEJM, with a brake of around 80% of this advanced cancer and makes it disappear in 16%. .

As far as Global Health is concerned, there are undiagnosed cases of tuberculosis (the missing tuberculosis), a disease that continues to have 10 million new cases a year and causes 1.5 million deaths. The Lancet report on what needs to be done to meet target 3.4 of (on non-communicable diseases) of the Sustainable Development Goals. Inequality in covid vaccination and its possible ways to solve it are the subject of an analysis by the NEJM. Concern for the health of the 3.5 million refugees from Ukraine. The low mortality of covid in Africa, a continent that was thought could be devastated by this disease, remains a mystery.

As for international health policy, the debate continues on the covid-zero policy in China, which has achieved great results so far (both in terms of mortality and economics), but it is not known if it will be able to continue with the variant “omicron”. The WHO judges the lifting of COVID measures in Europe too premature, considering that cases are growing, among other countries, in the United Kingdom, Ireland, Greece, France, Italy and Germany.

If we talk about national health policy (Spain), we are witnessing a relaxation of the measures against covid, despite its upward trend in recent days. Arrival of Paxlovid, Pfizer’s treatment for severe cases of covid. The important report published by the Catalan Society of Health Management, with an exceptional team led by Jordi Varela, on the forms of contracting health services from the point of view of value, is very noteworthy. The deans of Medicine react to the Ministry’s announcement of the increase in the “numerus clausus” in medicine by 10%.

In the field of companies, it is worth highlighting the evolution of the purchase of the fertility company IVI, the largest operation carried out by private equity companies in healthcare in Spain and with a company valuation of around 2,500 million euros.

SUMMARY

From the point of view of biomedicine, to highlight the new treatment for malaria in children; the new variant “Delatacron”, similar to the “ómicron”; and, the possibilities of artificial intelligence to accelerate the appearance of new medicines. We would like to underline the studies promoted by Valentín Fuster on ultrasound of the neck and groin arteries and their possibilities of detecting the risk of myocardial infarction and stroke to be promising, although it is a topic that has been talked about for a long time without much results.

Regarding Global Health, an interesting editorial in the British Medical Journal, entitled climate change, pandemic and war: an uncontrolled multi-crisis, since it seems that in this decade of the 21st century all the problems have accumulated; the verification that the low level of vaccinations in Africa is not so much due to the low level of vaccines, but rather due to the difficulties of reaching the population; interesting report on Nigeria and the problems of its health system.

In terms of international health policy, the problems of Asia’s “covid zero” strategy are noteworthy, which gave so much political return to the Chinese regime, presenting its low mortality successes and comparing them with the high mortality rates of other countries in the world ( United States, Brazil, United Kingdom, India, etc.). It seems that the “ómicron” variant, due to its contagiousness, is not easily approachable with the “covid zero” strategy, since that would lead to massive confinements in China. Resurgence of covid cases in Europe and Asia, which coincides with a relaxation of control measures.

If we talk about national health policy (Spain), it should be noted that the incidence of covid is growing, while the measures are relaxed, although there is concern in the Ministry of Health about a possible new wave of covid at the gates of Easter. Very important COFARES report on the pharmacy sector, proposing many innovative measures, which contrasts with the usual narcissism and laudatory speeches about the “Mediterranean model of pharmacy”, so liked by some professional representatives. Primary Care is trying to get back to normal, although with a thousand fewer doctors than before the pandemic. The birth rate in Spain is at historic lows, which is a serious problem. The Council of Dentists, in reporting that 34 million Spaniards have cavities, presents us with the reality of poor dental care in our country. A revolution in the use of biosimilars is coming, given the large number of biological medicines that will lose their patent in the coming years.

In the field of companies, it should be noted that Pfizer and BioNTech have requested authorization in the United States for a second “booster” for older Americans. Grifols bets on a vaccine against Alzheimer’s. HIPRA, the veterinary pharmacy company until now, hopes to start marketing its vaccine against covid in June.

Summary

From the point of view of biomedicine, the concern regarding resistance to antibiotics continues, after the extensive report published by The Lancet. This pandemic produces more deaths than AIDS and malaria. New variant of “ómicron” appeared in Denmark.

With regard to global health, the tenth anniversary of the declaration on “Neglected tropical diseases” should be highlighted, which provides an opportunity to reflect on a change in which progress has been made, but much remains to be done. COVAX does not have the capacity to receive more donations in the absence of basic financing for the purchase of syringes and other materials, essential to administer the vaccine.

As for international news, the WHO will publish a plan for the transition of the pandemic to the “control phase” in February. IHI initiative, of the European Union, to create a public/private platform for drug research.

In national news from Spain, the sixth wave seems to be starting to subside in terms of new cases, but still with high mortality. Interesting report promoted by Vitaldent on the public financing of dental care. Biosim highlights the savings potential of biosimilars in 2022. The issue of professional recertification advances, with a leading role for FACME.

In company news, the approval by the EMA of Paxlovid, Pfizer’s antiviral pill against COVID, is important. Also the large expected growth in turnover by COVID vaccine manufacturers in 2022 (29%, compared to 2021). Boom of online psychology platforms

SUMMARY

From an international point of view, the article published in The Lancet should be highlighted, in which mortality due to antimicrobial resistance, one of the most important causes of death in the world, is analyzed at a global level. Also the demand of the British Medical Journal that the scientific community have access to all the original data on the clinical trials of vaccines, since at this time these data are only accessible to employees of pharmaceutical companies. The Economist analyzes if the mandatory nature of the vaccine works, concluding that it does. It looks like all the COVID restrictions in England are going to end soon. The European Union gives more powers to the EMA, in case of having to manage future health crises.

As for the national news, the pandemic is still unchecked, in very high numbers and with high mortality; the debate on treating the covid as influenza continues, with opposing positions, those who are against it and those who defend it; the Ministry of Health wants to advance in the recertification, in dialogue with the FACME and the Organization of Medical Orders; The situation of health insurance in 2021 is presented, which did not stop growing, until the sector as a whole reached a turnover of close to 10,000 million euros; the Madrid College of Physicians signs a controversial agreement with UNIPROMEL, in theory to “defend doctors in free practice”, but, more possibly, with the intention of reinforcing the current weakened board of directors.

As far as companies are concerned, perhaps the most outstanding news is the sale by IBM of Watson Health, the artificial intelligence solution, designed to solve diagnostic and therapeutic problems, which had had so many problems in some centers such as MD Anderson from Houston. Although it was a good and ambitious idea, it was possibly ruined by short-termism, hyperselling and not taking enough into account the complexities of medicine and medical practice.

SUMMARY

From the international point of view, several issues stand out. In the first place, the agreement of the French Assembly taken on Sunday, January 16, to prevent the access of the unvaccinated to leisure and restaurant premises; also Quebec’s initiative to introduce special taxes on the unvaccinated; the WHO’s opinion that it is too early for COVID “flu” policies, as proposed by Spain; the calculation by ISGlobal (Barcelona) that COVID has already left 17 million deaths globally; and the big push for research on neurodegenerative diseases.

As for national news, the SEMFYC (one of the Primary Care medical societies) editorial had a great impact defending the “flu” policies of COVID and, therefore, the minimization of measures, encouraging, consciously or unconsciously, the “light” negationism. This position was quickly adopted by the central government and certain authorities of the Community of Madrid. The central government even tried to lead this position at the international level. Rejection of this idea by other Primary Care societies, a large number of experts, the WHO and the EU. Meanwhile, the incidence of COVID continues to rise to figures never seen in other waves (2,923 cases per 100,000 inhabitants). The price of antigen tests is regulated, after multiple requests from various groups. It is agreed to extend the third and fourth doses. Serious problems regarding sick leave, for which solutions are articulated about the legality of which is in doubt. The purchase of COVID medicines is announced. Ayuso Plan to stimulate the birth rate in Madrid.

As far as companies and other actors are concerned, a group of scientists in Texas announce a vaccine against COVID, which, according to them, it will be cheaper, easier to make and patent-free. Pfizer announces that it will have a new vaccine against the “ómicron” variant ready in March

SUMMARY

From an international point of view, perhaps the most striking thing is the great global expansion of COVID, particularly of the “omicron” variant, with a record number of infections in the United States, of more than one million a day.

As for national news, official policy continues to trust everything to vaccination, avoiding other measures that may have certain political costs. The problem is the number of deaths, the collapse of the health system (which affects not only COVID cases, but the rest of the patients) and the large number of sick leave. The only measure at the national level is the obligation of the mask outdoors (about whose effectiveness many doubt), without addressing the need to control the aeration of premises based on CO2 meters; the avoidance of crowds; considering the mandatory nature of the vaccine for certain groups (health workers, teachers, etc.); the more agile activation of the third dose; ensuring the existence of antigen tests and regulating their price; the management of sick leaves in another more agile way, in such a way that Primary Care no longer completely collapses; and, ensuring the purchase and accessibility of new drugs against COVID.

Regarding company news, to highlight the record sale of 7 million antigen tests by pharmacies in the week of Christmas; and the great news that the Spanish HIPRA vaccine may be present in the second quarter of year 22.

Summary

From an international point of view, perhaps the most striking thing is the great growth worldwide of the “omicron” variant, already present in 89 countries. Although it is clear that it is much more transmissible than the Delta variant, it is not clear that the disease is milder. Some countries in Europe decree confinements. The effectiveness of COVID passports is demonstrated, in terms of increased vaccinations. Figures for healthcare spending in the United States are released in 2020, spending increased significantly, especially federal spending, related to COVID. It reaches 19.7% of GDP (which is said soon). It is also striking that for the first time in the history of the United States, public health spending exceeds private spending.

Regarding national news, great concern about the rapid expansion of the pandemic, which forces the President of the government to convene a Conference of Presidents, for two days before Christmas Eve, which prevents measures from being adopted for Christmas. The plenary session of the Interterritorial Council approves the “Primary and Community Care Plan 2022-2023”, a set of good intentions and the announcement of measures that, where appropriate, would have to be implemented by the autonomous communities. Once again you see how difficult “co-governance” without good governance.

In relation to the activity of companies and other agents in the sector, the activity of vaccine manufacturing companies continues. Two things are difficult to understand: the letter from the President of the Madrid Medical Association to the President of ASISA and the launch by DKV-Yoigo of a medical insurance for 20 euros. We believed, on the one hand, that health insurance companies were free to hire their medical staff; and, on the other hand, that it was difficult to continue lowering the price of health insurance in our country. It is true that in the United States and other countries online insurance, Oscar type, is emerging with cheaper policies than conventional insurance. But in Spain the policies are already very cheap and I do not think that it is about making policies for young and healthy people, who hardly consume health care.

SUMMARY

From the international point of view, it is worth highlighting the beginning of the work of the WHO for the approval of a Treaty on pandemics. It seems that the alarm about the new “‘ ómicron “variant is lowered, after the WHO announcement that there had been no case of death. We must be attentive to the program of the new German coalition government, and its impact on the health system. It seems that its actions are concentrated in the pharmaceutical sector. The failure, for various reasons, of the COVAX platform for immunization of low- and middle-income countries is confirmed.

At the national level, the growth of COVID continues to figures that are already beginning to be worrying, despite the advantage over other European countries. The council of ministers approves the Mental Health Strategy, a plan that, unlike the one approved a few years ago on Primary Care, does have funding, despite having aroused reservations among experts. The council of ministers also approves the PERTE for health. As of December 13, the vaccines for children will be available.

Regarding companies, it should be noted that both Pfizer and Moderna are preparing to adapt their vaccines to the new COVID variant.

ABSTRACT

From the international point of view, the plan of the American Democrats to negotiate the price of medicines and its possible impact on innovation continues to be discussed; Europe is once again the engine of the pandemic, with demonstrations in some European cities against confinements and COVID passport measures; and finally, the United States reach a peak in mortality from opioid as the covid pandemic continues.

At the national level, four elements of interest: 1. Spain exceeds 100 cases per 100,000 inhabitants, although the situation is worrying, nothing to do with that of other European countries; 2. The state-of-the-art health PERTE is presented, doubts regarding its reformist impact; 3. Publication of the draft law that aims to consolidate the universality and equity of our National Health System; 4. The waiting lists are published as of June 30, 2021.

In terms of company news, perhaps most relevant is that both Merck and Pfizer’s COVID therapies will be available in generics for low- and middle-income countries.

NATIONAL

  • Spain exceeds 100 cases per 100,000 inhabitants for the first time in two months (El País, 18 Nov 21). Comment: although the situation is worrying, it has nothing to do, however serious, with that of other parts of Europe. It seems that the high immunization, the fifth wave and the high percentage of vaccination are at the origin of this situation. However, we do not know if this differential is going to be maintained or we are heading towards a situation similar to that of most of the rest of Europe.

Comment: there is no transformation in the sense of reforming the health system (which they consider good) and it is an investment aimed at the biotechnology industry. Mentions of primary care are imprecise. In summary: facing the transformation of the health system and to what extent European funds can help, including digitization, is an issue that remains to be seen
