7 days in healthcare (November 21st-27th, 2022)

Summary

Most notably from the Biomedicine standpoint, the FDA is approving a drug that could delay the onset of type 1 diabetes, the devastating disease that often occurs in adolescents. Also the preclinical study that seems to foreshadow a universal flu vaccine. Likewise, the minirobot designed by Apple capable of taking medicines to the brain, that organ until now practically inaccessible to drugs.

With regard to Global Health, the COP27 summit is considered in an article in The Lancet a “collective failure”, given the lack of agreement on the lower use of fossil fuels. We must also note the race for the production of vaccines in several African countries.

Regarding International Health Policy, mortality from covid remains high and is very worrying in some countries, mainly Japan, Brazil, India, China, the Russian Federation, South Korea and Spain. The covid-zero policy is generating problems and discontent in a country where public demonstrations are as controlled as in China. The nurses’ strike in the United Kingdom is a historic event in that country, which will undoubtedly will have consequences.

If we talk about National Health Policy (Spain), mortality from covid remains high in Spain (260 cases in the last week). At this time Spain has 0.6 deaths per 100,000 inhabitants, a fairly high mortality, although lower than other European countries, such as France, Greece, Denmark, Norway, Croatia, Estonia and Luxembourg, according to WHO data. The waiting lists for June 30 are published by the Ministry of Health. A real national problem and a serious delegitimization of the public system, which some minimize. The strike of Primary Care doctors continues in Madrid, while the conflict threatens to spread to other autonomous communities. The rise in the electricity bill threatens the profitability of private hospitals, according to ASPE.

In the field of Companies, at the international level, Amazon is making a new foray into healthcare, this time with the “Amazon Clinic” platform, a telemedicine service that will be extended to 32 American states and offers virtual care for 20 clinical conditions. At the national level, news at HM Hospitales, with the new hospital in Rivas; in Recoletas, which extends to Salamanca; and, in Sanitas, which intends to create its own clinical analysis laboratory.

    • Amazon makes a new attempt at healthcare. The offers of the big technological platforms are diversified: Apple tries to monitor through the iPhone; Microsoft, offers computer services to companies; Alphabet (Google), relies on wearables. 7 days in healthcare (September 26th-October 2nd, 2022)

Summary

From the point of view of Biomedicine, to review the advances, still to be confirmed, in the treatment of two terrible diseases: ALS and Alzheimer’s.

Regarding Global Health, concerns continue about the evolution of polio, whose eradication was announced in 2019, but which is reappearing in several countries.

Regarding International health policy, it is worth noting the similarities between the covid-zero policy in China and the era of prohibition in the United States, something pointed out by The Economist. Very worrying is the “burnout” of doctors, a phenomenon highlighted by the New York Times in the United States, but which seems to be universal.

If we talk about National health policy (Spain), the debates on the Equity Law continue. The update of the RD on the minimum data set of the SNS leaves out the private sector, despite the opposition of the POP, IMAS Foundation, IDIS Foundation and ASPE. It is a modification of a 2010 RD, but in twelve years the private sector has developed a lot and it is no longer so easy to leave it out in certain decisions. The Senate rejects the PP’s motion to facilitate access to medicines and reduce the time it takes to join the SNS. Surely the motion of the PP was too vague and, at this point, it is necessary to explain very well the “what” and the “how” of any proposal.

At the corporate level, at the international level, big rise in Biogen shares, spurred by what appear to be hopeful results from its Alzheimer’s drug. At the national level, there are rumors that KKR and CVC could be trying to buy Quirónsalud, currently in the hands of the German group Fresenius, which has recently changed its CEO. The Murcian company Vócali develops the Inbox Medical software for transcription of medical reports.

Summary

From the point of view of biomedicine, immunotherapy (in colon cancer) and CAR-T therapies (in lupus) continue to expand their therapeutic possibilities. Advances against infections for resistant bacteria. Will there ever come a time when cancer can be diagnosed by a simple blood test? This possibility does not seem very remote, according to papers presented at the meeting of the European Society of Medical Oncology.

Regarding Global Health, an important work directed by Jeffrey Sachs and published by The Lancet on the delays and errors of both governments and the WHO in dealing with the covid pandemic, which could have produced 17.7 million of  deaths, many of them preventable.

Regarding international health policy, the WHO says that the end of the covid pandemic may be in sight, although at the same time a new variant (BJ.1) of covid is evident in Europe. Important debate in Europe (Scotland and France) on euthanasia and assisted death. The French Ethics Committee opens the door to this possibility. The financing of the American FDA was questioned, due to its great economic dependence on the pharmaceutical industry. Biden launches a speech commemorating Kennedy’s in 1962 when he decided to go to the Moon, but this time oriented towards the fight against cancer.

If we talk about national health policy (Spain), the PDeCAT presents an amendment to the entire equity law, which it accuses of interfering in regional powers and of confusing agreement with privatization. Surprising statements from the Ministry of Health in a parliamentary response, saying that its functions in relation to the waiting lists are only to inform and monitor, as if it did not have the possibility of signing agreements with the communities to address this problem or of launching other actions. Concern, there are even demonstrations in the street, of the relatives of the elderly admitted to residences. The Community of Madrid announces that it will reduce the waiting lists by half. Although we would like it not to be the case this time, this announcement is preceded by other similar ones from different governments, which always came to nothing in terms of effectiveness. The search for solutions by the Ministry of Finance to the MUFACE model, highly impacted by inflation, seems to recognize its underfinancing.

In the field of companies, from the international point of view, important success for Sanofi, with its new vaccine against bronchiolitis. In Spain, important corporate movements in Korian, HM Hospitales, Miranza and Ribera.

Summary

From the point of view of biomedicine, to highlight the Science article, showing that the brain and the immune system are interconnected. Monkeypox is transmitted by direct contact, therefore “ring vaccination” is indicated, only to contacts of an infected person.

As far as Global Health is concerned, the universal coverage approach in Nigeria, the most populous country in Africa, should be highlighted. It is seen that universal coverage is no longer just something of Europe or the industrialized countries.

As for international health policy, the frequent shootings in American schools are covered by medical journals (The Lancet, British Medical Journal), as it is also a health problem. Something perfectly avoidable.

If we talk about national health policy (Spain), it should be mentioned that private health offers to collaborate with public health in addressing the problem of waiting lists.

In the field of companies, Takeda’s statements are surprising, saying that pharmaceutical companies will be able to lower prices in the face of a global crisis. The acquisition of the HIPRA vaccine by Brussels is advancing.

Summary

From the point of view of biomedicine, the important study published by the journal Science should be highlighted, outlining the most detailed atlas of human cells to date. The discussion about the origin of acute childhood hepatitis continues.

As far as Global Health is concerned, it is found that the United States has reached one million deaths from COVID and Europe two million. A sad milestone.

Regarding international health policy, in relation to COVID, the debate continues on the sustainability of the zero-COVID policy in China. Lockdowns in North Korea, possibly the most opaque country in the world. The debate continues in the United States on the modification of the Roe v Wade sentence. Pronouncements from scientific journals (Nature, The Lancet), as well as from the President of the American Medical Association. The ECDC (European Center for Disease Prevention and Control) stops recommending the use of masks on public transport in Europe.

If we talk about national health policy (Spain), the accumulated incidence of COVID continues to grow. Great debate regarding the announced new Abortion Law. It is more than doubtful that the Law will be approved and, if it is, that it will be complied with, given the large number of controversial elements (registration of conscientious objectors, the obligation that these interventions be carried out in public hospitals, the use of terminology of obstetric violence, etc.). The Ministry of Industry takes out the first tender of the cutting-edge health PERTE. Important editorial in El País on waiting lists, which it presents as a scourge of the SNS.

In the field of companies, the WHO denounces attempts to whiten tobacco companies, by way of presenting themselves as green companies.

ABSTRACT

From an international point of view, to highlight something we knew: that WHO, in its current configuration, is not prepared for global health challenges. However, this finding is important in the article in the British Medical Journal by Anthony Costello. Europe, once again in the center of expansion of the pandemic. The decision of the North American Democrats to reduce the price of medicines, via the negotiation in Medicare, is very far-reaching. This decision, if applied, will have major consequences for the entire pharmaceutical industry globally.

At the national level, to highlight the article in The Lancet, which says that Spain could have achieved group immunity. For the rest, the incidence of COVID continues to grow in our country, although it does not reach, by far, those of other European countries. Waiting lists are a big concern throughout Spain. Controversy over the new Equity, Universalization and Cohesion Law. Podemos wants to take advantage of it to repeal Law 15/1997, which allows the outsourcing of health services.

As a business activity, to highlight the two new antivirals against covid-19, one from Merck and the other from Pfizer. Merck has released the patent so that it can be used widely in developing countries. Novartis dissociates itself from Roche, selling it its 33% stake in the latter company.

INTERNATIONAL

  • The WHO, in its current configuration, is not ready for its purpose, but is limited by lack of funds and political interference, and this has been demonstrated in this pandemic that has produced more than 18 million deaths, writes the economist Anthony Costello in the British Medical Journal (BMJ, 3 Nov 21)
  • Europe once again at the center of the covid pandemic, says WHO, (The Guardian, 4 Nov 21)
  • The WHO warns of the increase in deaths from covid in 56 countries, and insists that vaccine manufacturers have not done their job well (ConSalud, 5 Nov 21)
  • Democrats in the United States reach an agreement to reduce the price of drugs, allowing the government to negotiate the price of Medicare drugs. The United States pays three times more for many drugs than Europe, according to a study by the Brookings Institution. Initially, the measure will be launched with the 12 most expensive drugs (FT, 2 Nov 21)
  • The fourth wave continues to beat the record of infections in Greece, with almost 7,000 daily (El Confidencial, 6 Nov 21)
  • Excess mortality grows in Russia, since the beginning of the pandemic (FT, 2 Nov 21)
  • The management of the pandemic leaves a trail of investigated political leaders: Brazil, France, Italy, Austria, India, … (El Debate, 2 Nov 21)
  • India’s successes against covid are analyzed: more than half of Indians have received at least one dose of the vaccine. 6 million are vaccinated every day (The Economist, 29 Oct 21)

NATIONAL
